I'm a resident in Neurotology at a prestigious institute in Mexico City. When I first saw the two huge volumes of this book, I thought it was going to be dull and full of unaccesible information for a beginner such as myself. I was gladly surprised with the refreshing style and easy-to-follow introductory chapters of Anatomy, physiology and Pathology (including molecular genetics applied to the specialty). Every detail is fully explained, including physics when necessary. This is an all-in one, it goes from basics, including all the main tests used in neurotologic diagnosis, pediatric neurotology, imaging diagnosis (with my personal favorites on imaging of the facial nerve and cerebellopontine angle), surgical features,and any disease related that you may expect to find. What I found most appealing is that I've been able to find basically everything I need on a subject, without filling my desk with different books that include ONLY tests and diagnosis, or ONLY Vertigo, like if it was all there is in neurotology. Taking a look at the long list of contributors gives you a hint on why this is a MUST for the specialist in Neurotology.

Preface:
Alzheimers disease (AD) is characterized by the deposition into the brain of amyloid
peptide, which originates a cascade of inflammatory events leading eventually to neuronal
death. These pathological events likely occur several years before the clinical manifestation
of the disease, implying that potential therapeutical interventions are currently started too late
to give beneficial results. Recently, growing attention has been paid to the Mild Cognitive
Impairment (MCI), considered the prodromal phase of AD, as 80% of subjects with MCI
have been shown to develop AD within 5 years. Several studies aimed to identify biological
markers to differentiate between normal aging and incipient AD have been carried out,
including cerebrospinal fluid (CSF) analysis and neuroimaging. In particular, the evaluation
of CSF Amyloid beta (1-42) levels, together with tau and phospotau, are of help for
recognizing early AD. Besides, a number of additional molecules are altered in CSF. Other
early modifications have been observed in peripheral cells, such as fibroblasts and
leukocytes, as well as in serum from patients. Biomarkers for AD represent important tools
supporting the clinical diagnosis and the choice of potential therapeutic options. Moreover,
they would be of great help for the selection of cohorts of homogeneous patients for clinical
trials with new disease-modifying compounds.
This book is aimed to give an update about MCI as prodromal Alzheimers disease (AD),
to discuss the main known pathological mechanisms at the basis of AD and to describe the
possible biomarkers to be used for an early diagnosis of the disease.

Stroke increases the risk of dementia by approximate 2-9 times. World wide, vascular dementia (VaD) is the second commonest cause of dementia. Presence of post-stroke dementia (PSD) reduces survival among stroke survivors and increases risk of long term stroke recurrence. Varying severity levels of cognitive impairment affects functional outcome independent of physical impairment. With an ageing population, burden of PSD is expected to rise. Although PSD is prevalent, relevant to mortality and morbidity, and is potentially more preventable than Alzheimer's disease (AD), it has received less attention relative to post-stroke physical impairment or to AD. It is not until the last decade that more attention has been paid to PSD. Historically, in the late 19th century, VaD was considered to be the commonest cause of dementia. It was thought that dementia was due to a relentless strangulation of the brain's blood supply, resulting in degeneration of the brain and dementia.In the middle of the 20th century, pathological studies have begun to reveal that senile plaques and neurofibrillary tangles, rather than vascular lesions, were key findings among most of the elderly demented patients with atrophic brains. In the early 70s, VaD was thought to be uncommon and the mechanism of VaD was attributed to multiple small or large cerebral infarcts (multi-infarct dementia). In recent decades, advances of neuroimaging have enhanced greatly our understanding on VaD, its complexity far exceeds that of the concept of multi-infarct dementia. The authors aim to review the current understanding of PSD and its imaging. Poststroke dementia will first be defined. Following that, foundational issues of PSD including its classification, pathology, prevalence, and cognitive pattern will be reviewed. This sets the background for discussing the imaging aspects relevant to PSD.
